How far to customize the images?

It really is a personal choice. Some of the provided graphics will work for a basic skin, but in all honesty...this seems very amateur to most people. The visual appeal of a forum, believe it or not, is an important thing when you are trying to gain more people. I, myself, have avoided joining forums in the past simply because they only used a color scheme and didn't change any of the images. I think you have a great start here and maybe not all of the images are need to be changed. It really boils down to what others who are contemplating joining will think. Some don't care about the overall look, but I think you'll find the majority of people do. Hope this helps. Also, I didn't vote in the poll, because....well...I think you know what I'd vote for: I make full and complete templates.

I voted based on what I would do.

I like to customize every single image that I can find... hence why my forum has even the tiny little pixel icons/buttons customized as well, from the main banner and background right down to the smilies and profile icons. (which took forever, I might add since I created them from scratch, haha) Basically everything that you are allowed to add your own URL image to in the "Customize Images" area in the Admin panel is filled.

But like Pooky said, it's a matter of personal choice too. It also depends on what level you are currently at with graphics.
